Most insurance plans do provide some coverage for custom made orthotics as well as Chiropody or Podiatry treatments. Every insurance plan is different, so we strongly suggest contacting your insurance company before your appointment to determine exactly what is covered by your plan.

Making insurance claims can be a confusing process, that’s why we’ve created this handy guideline for you to make things easier when submitting all your documents:

Orthotics Assessments and Footcare Treatments:

(2 documents)

1. Stripe Receipt

This is like your credit card slip, similar to what you’d get at a store and is emailed to you after you book your appointment


2. HealthCasa Treatment Receipt

Found in your Past Appointments immediately after your appointment



Orthotics:

(5 documents)

1. Stripe Receipt

This is like your credit card slip, similar to what you’d get at a store and is emailed to you within 24 hours after your appointment.


2. Orthotics Prescription (Rx-OT)

This is your orthotics prescription created for you by your podiatrist/chiropodist. Found in your Past Appointments (Documents folder) within 24 hours after your appointment.


3. Biomechanical Assessment/Gait Analysis Report (BA)

Found in your Past Appointments (Documents folder) within 24 hours after your appointment.


4. HealthCasa Orthotics Receipt

Found in your Past Appointments when you receive your orthotics


5. Lab Report & Lab Invoice (2 page document)

Attached to your HealthCasa Orthotics Receipt.
